Band Parents,

Just so you can be planning on taking care of this over the summer break, this is a reminder that every year, all band members need a physical form completed in order to participate in Band, just like athletes. This is the same form that is used for athletics (so, if your son or daughter has already completed this for athletics, you will also need to give a copy to the Band Directors for their files).

There are a number of places that will conduct this physical for about $15, or you can have a full physical done through your child's M.D.
